 The Practice welcomes new patients at all our surgeries. Please fill out the following forms in full so we can process you application swiftly: Family Doctor Services Registration Form (available here) New Patient Questionnaire If you are transferring directly from another GP surgery in the UK then you may register by post. If not then you will need to register at the surgery in person and bring some ID with you. Forms of ID accepted are Passport, Driving licence, Birth certificate, Marriage certificate or Identity card. Please make sure you indicate on the New Patient Questionnaire which surgery you wish to join.
